Many student insurance plans include prescription drug card benefits. They may appear to be part of the plan but are actually a separate
program. Prescriptions must be filled at a participating pharmacy. Claim forms are not necessary. The prescription drug card company negotiates
with national and local pharmacies to sell you medications at a discount below their retail price. You pay a minimal co-pay (usually $10 or
$20 - depending on whether it is a generic or brand name medication) at a participating pharmacy. The balance of the discounted amount is
charged against your student insurance plan benefit maximum by the pharmacist.
If your plan includes a prescription drug card benefit, you will be issued an ID card to show proof of coverage to the pharmacy.

During the first several days of your coverage, until we have an opportunity to report your name to the prescription drug card company,
their national pharmacies do not know you are part of the plan. During this time you must pay for medications and submit a special Direct
Reimbursement Claim Form for reimbursement. These claim forms can be obtained at your student health service or by contacting Administrative
Concepts, Inc. at (888) 293-9229.
Not all medications are covered. Typical exclusions include birth control pills, acne treatments and vitamins. A list of covered
medications and exclusions is included with your ID card.
